Updated May 2026
If you’re an international medical graduate (IMG) looking to pursue residency training in the United States, obtaining your ECFMG certification is a vital step in the process. This certification not only verifies that you meet the standards required for U.S. graduate medical education (GME) programs but also makes you eligible to apply for residency positions. Note that while ECFMG continues to handle certification, exam registration for Step 1, 2, and 3 moved to the FSMB directly for IMGs as of January 2026; so certification and registration are now handled by separate organizations.
A critical aspect of ECFMG certification is fulfilling the clinical and communication skills requirements. With the discontinuation of the USMLE Step 2 Clinical Skills (CS) exam, the ECFMG now offers six pathways to satisfy these requirements. Understanding these pathways (especially Pathway 1 and Pathway 6) will help you determine which is the best route for you. This blog will provide a comprehensive overview of these pathways, along with key information on certificate expiration, how to make your certificate permanent, and important deadlines.

2027 ECFMG Pathways Updates for Match 2027: New Rules, Deadlines & Requirements
ECFMG has rolled out the 2027 Pathways for ECFMG Certification (current as of May 14, 2026), and if you’re aiming for Match 2027 you need to know what changed. Here’s the quick-hit summary before we get into the full breakdown:
- Pathway 1 license date shifted: Your unsupervised license/registration must have been valid at any time on or after January 1, 2022 (was January 1, 2021 for the 2026 cycle).
- Application fee increased: $945 USD for the 2027 cycle (up from $925), non-refundable, credit card only.
- Hard deadlines for Match 2027: Pathways application + all required documentation + OET scores must be received by ECFMG by January 31, 2027 (Eastern Time). For Pathway 6, all six Mini-CEX evaluations must be submitted by February 15, 2027. Take OET on or before the last available test date in December 2026.
- Application portal: Submit your 2027 Pathways application through MyIntealth at pathways.myintealth.app. Once you start an application, you have only 5 days to complete and submit it before it’s deleted.
- Pathway 6 (15-day acceptance window): If a physician evaluator doesn’t accept your request within 15 days, it’s automatically withdrawn. After they accept, you have another 15 days to complete the encounter and have them submit the Mini-CEX. Plan accordingly.
- Pathway 6 (Gold/Platinum USMLE ban): ECFMG will NOT accept Mini-CEX evaluations from students of, or evaluators associated with, Gold USMLE, Platinum USMLE, Dr. Jino Thomas, or any successor corporation.
- Pathways Revalidation OET requirement: If you are revalidating an expired Pathway, your satisfactory OET Medicine score must be from a test taken on or after January 1, 2025. Older scores no longer count for revalidation.
- New certificate expiration tier: 2027 Pathways certificates expire December 31, 2029 (unless you obtain permanent validation through 12 months of ACGME training or an unrestricted U.S. license).
What are the general requirements to obtain an ECFMG certificate?
To obtain an ECFMG certificate, international medical graduates (IMGs) must meet the following requirements:
- Pass USMLE Step 1: This exam assesses your knowledge of basic medical sciences.
- Pass USMLE Step 2 Clinical Knowledge (CK): This exam evaluates your clinical knowledge and application in medical practice.
- Satisfy Communication Skills Requirements: You must pass the Occupational English Test (OET) Medicine, which assesses your ability to communicate effectively in a healthcare environment.
OET Medicine is required for all applicants (even US citizens, native speakers, or those trained in English). Whether you are submitting a new application or revalidating an expired Pathway, your OET Medicine score must be from a test taken on or after January 1, 2025 (older results, even if previously accepted, will not count). For Match 2027, take OET Medicine on or before the last available test date in December 2026 to ensure your scores reach ECFMG in time. ECFMG must receive your OET scores directly from OET no later than January 31, 2027. - Complete a Pathway: You must fulfill the clinical skills requirement through one of the six ECFMG pathways, which vary based on your specific qualifications, such as holding a medical license or undergoing evaluations by licensed physicians.
- You must follow the pathways in order (1→2→3→ 4→5→6), unless you failed Step 2 CS, in which case you go directly to Pathway 6.
Meeting all these requirements is essential to receiving ECFMG certification, which allows you to apply for U.S. residency programs.
Pathway 1: Already Licensed to Practice Medicine in Another Country
Pathway 1 is ideal for IMGs who already hold or have recently held an unsupervised license to practice medicine in any country.
Eligibility Requirements for Pathway 1 for ECFMG Certification
To apply for Pathway 1, applicants must meet the following criteria:
- Unsupervised Medical License: You must have held a license to practice medicine without supervision in any country on or after January 1, 2022. This unsupervised license is a key requirement, as training licenses or supervised practice licenses do not qualify for Pathway 1.
- License Validity: Your medical license does not need to be currently active at the time of application. However, it must have been valid at any point on or after January 1, 2022. For example, if your license was valid in 2023 but has since expired, you are still eligible to apply through Pathway 1.
- No Disciplinary Actions: The license must not have been subject to disciplinary actions. If your license was revoked or suspended due to professional misconduct or other issues, you will not be eligible for this pathway.
- Satisfaction of Local Regulatory Requirements: You must have met all legal and regulatory requirements to practice medicine in the country where the license was issued, including registration with the appropriate Medical Regulatory Authority.
- Step 2 CS Consideration: If you have failed the Step 2 CS exam one or more times, you are not eligible for Pathway 1. Instead, you must apply through Pathway 6.
Application process for Pathway 1
If you qualify for pathway 1, you will need to provide ECFMG with documentation from the issuing authority of your license. The preferred method is to request a Certificate or Letter of Good Standing/Current Professional Status from the licensing authority, which must be sent directly to ECFMG. This document verifies that your license met the unsupervised practice requirements and was not subject to disciplinary actions.
Documentation must be issued within 90 days of submission, and it’s critical that you plan ahead to ensure this verification process aligns with your application timeline. You can read more details about pathway 1 here.
NEW for 2027: The 2027 Pathways application is submitted through the MyIntealth portal at pathways.myintealth.app. Once you start the application, you only have 5 days to complete and submit it before it’s deleted; so make sure your documentation is lined up before you log in. ECFMG accepts three documentation methods for Pathway 1:
1. Option A (preferred): Have your licensing authority email a Certificate or Letter of Good Standing/Current Professional Status directly to licensure@ecfmg.org. Do not email this address yourself; ECFMG will only respond to messages from authorities.
2. Option B: Upload the document yourself when you submit your application. ECFMG will primary-source verify it, which can delay your application.
3. Option C (last resort): If your MRA won’t cooperate with either of the above, upload your license itself and ECFMG will attempt to verify on your behalf, but this is not guaranteed to succeed, so push hard for Option A first. The verification must reach ECFMG by January 31, 2027 for Match 2027.
Pathways 2-5: Medical School Specific Pathways
Pathways 2 through 5 are intended for applicants from specific medical schools that meet certain accreditation standards or provide clinical exams required for licensure.
- Pathway 2: For graduates of schools that administer an Objective Structured Clinical Examination (OSCE) required for licensure.
- Pathway 3: For those from schools accredited by an agency recognized by World Federation for Medical Education (WFME).
- Pathway 4: For graduates of schools accredited by an agency that has received a Determination of Comparability by National Committee on Foreign Medical Education and Accreditation (NCFMEA)
- Pathway 5: For graduates of joint degree programs with a U.S. medical school accredited by the Liaison Committee on Medical Education (LCME).
- Graduation time consideration: must be on or after Jan 1, 2024 for the 2027 cycle (Verify your specific graduation date on the Pathway 2/3/4/5 page of ecfmg.org since requirements roll forward each Pathways season).
You can check if your medical school qualifies by using the list provided on the ECFMG website here.
Pathway 6: Evaluation of Clinical Patient Encounters by Licensed Physicians
Pathway 6 is the most complex of all the pathways, designed for IMGs who do not qualify for Pathways 1 through 5. It involves having your clinical skills evaluated by licensed physicians in real-world clinical settings. The ECFMG Mini-Clinical Evaluation Exercise (Mini-CEX) is used to assess your performance across six in-person patient encounters.
IMPORTANT NOTICE (NEW for 2027): ECFMG does NOT accept Mini-CEX evaluations from students of Gold USMLE, Platinum USMLE, or any successor corporations. ECFMG also does not accept Mini-CEX evaluations from physician evaluators known to be associated with Dr. Jino Thomas, Gold USMLE, Platinum USMLE, or any successor corporation whether the evaluator is currently known to ECFMG or is discovered later. If you use any of these companies to source your evaluators, your application will be rejected and you may face a finding of irregular behavior. Vet your evaluators yourself and confirm they are independent.
Eligibility Requirements for Pathway 6 for ECFMG Certification
- Ineligibility for Other Pathways: You must first confirm that you are not eligible for Pathways 1 through 5. Pathway 6 is intended as a last resort for IMGs who need an alternative method to prove their clinical skills.
- Real In-Person Clinical Encounters: Your clinical skills must be observed and evaluated during six distinct real, in-person patient encounters. Virtual encounters, telemedicine, or standardized patient simulations are not accepted for Pathway 6.
- Licensed Physician Evaluators: Each encounter must be evaluated by a licensed physician who meets ECFMG’s criteria. Importantly, each physician can only evaluate up to two of your encounters, meaning you will need to identify between three to six qualified physicians to complete your evaluations.
Criteria for Physician Evaluators in Pathway 6
Selecting appropriate physician evaluators is one of the most crucial aspects of Pathway 6. To ensure the integrity of the assessment, ECFMG has set strict criteria for who can serve as an evaluator:
- Full, Unrestricted License: The physician must hold a full, unrestricted license to practice medicine without supervision in the country where the encounter takes place.
- Five Years of Experience: The evaluator must have held this unrestricted license for at least five years. Physicians who have been practicing for fewer than five years, even if fully licensed, do not qualify.
- No Relatives: The evaluator cannot be related to you. Relatives include, but are not limited to, spouses, parents, grandparents, children, grandchildren, siblings, uncles, aunts, nephews, nieces, and cousins.
- No Compensation: The physician cannot be compensated by you or any third party for their participation in the evaluation. They must conduct the evaluation voluntarily, typically as part of their regular medical duties.
Each physician can only evaluate up to 10 applicants per Pathways season for the Pathways program. If the evaluator has already reached this 10-applicant cap, or has opted out of Pathway 6 evaluations, the system will flag this when you enter their email and you will be unable to request that physician. It’s important to identify evaluators who meet these criteria early in your application process, as finding willing physicians who meet these qualifications can be challenging.
Criteria for Acceptable Clinical Encounters in Pathway 6
The clinical encounters themselves must meet strict guidelines:
- Real, In-Person Encounters: All encounters must involve real patients in a formal clinical setting and only outpatients (not inpatient/ER). Encounters conducted in a simulated or virtual environment will not be accepted.
- Primary Care Focus: Encounters must be in primary care or general practice, focusing on diagnosing and treating acute or chronic illnesses. Subspecialty encounters do not qualify.
- Patient Consent: You must ensure that the patient has given consent to be observed during the evaluation.
ECFMG Mini-CEX for Pathway 6
The ECFMG Mini-CEX is a structured assessment tool that physicians will use to evaluate your clinical performance. During each encounter, you will be evaluated on four core components:
- Medical Interviewing Skills: Your ability to gather relevant patient history and communicate effectively.
- Physical Examination Skills: Your competence in performing physical examinations.
- Professionalism and Communication Skills: Your interactions with the patient, maintaining professionalism, and clear communication.
- Clinical Reasoning and Judgment: Your ability to make sound clinical decisions based on the information gathered during the encounter.
Each component is scored on a scale of 1 to 9, with a score of 4-6 representing the level expected of a recent medical school graduate ready for postgraduate training. Scores of 7-9 indicate performance at the level of an experienced practicing physician.
Check a sample of the ECFMG Mini-CEX for Pathway 6 here.
Your overall score will be the average of all six encounters across these four components. If your score does not meet the minimum threshold, your evaluations will be reviewed by a committee, which will make a final determination on whether you have met the clinical skills requirement.
Application Process for Pathway 6
The application process for Pathway 6 involves several key steps:
- Submit Application: Begin by confirming your eligibility and submitting your application for Pathway 6.
- Identify Physician Evaluators: Enter the details of your physician evaluators into the application system. Each evaluator must accept your request for evaluation before the encounters can take place.
- Complete Encounters: After your evaluators have accepted your requests, schedule and complete the six in-person clinical encounters.
- Evaluator Submits Mini-CEX: After each encounter, the physician must submit the Mini-CEX evaluation electronically. You are responsible for ensuring all evaluations are submitted by the deadline. NEW for 2027: Once a physician accepts your request, you have 15 days to complete the encounter AND have the evaluator submit the Mini-CEX through ECFMG’s Clinical Skills Evaluation and Attestation Portal. If they don’t submit within 15 days of acceptance, the request is automatically withdrawn and you’ll need to add a new evaluator. Also, if a physician does not even accept your request within 15 days of submission, the request is automatically withdrawn. For Match 2027, all six Mini-CEX evaluations must be received by ECFMG no later than February 15, 2027.
- Final Review: Once all evaluations are submitted, ECFMG will conduct a final review to determine if you have met the requirements for certification.

Frequently Asked Questions about ECFMG Certification
When does my ECFMG certificate expire?
The expiration date of your ECFMG certificate is tied to the specific Pathway season in which your Pathway application was accepted even if your certificate has different expiration date from what is listed below. If you met the clinical and communication skills requirements through the Pathways, your certificate will expire based on the following schedule:
- 2027 Pathways: Expires December 31, 2029
- 2026 Pathways: Expires December 31, 2028
- 2025 Pathways: Expires December 31, 2027
- 2024 Pathways: Expires December 31, 2026
- 2023 Pathways: Expires December 31, 2025
- 2021 or 2022 Pathways: Expires December 31, 2024
Your ECFMG certificate will expire when your Pathway expires unless you take steps to revalidate it or meet the requirements for permanent validation. Revalidation is only possible through Pathways 1 or 6. So, if you’re not eligible for Pathway 1, you must apply through Pathway 6, which is considerably more difficult.
How can I make my ECFMG certificate permanent and avoid expiration?
To make your ECFMG certificate permanently valid and avoid expiration, you must successfully complete at least 12 months of clinical education in a U.S. graduate medical education (GME) program accredited by the ACGME, or in a non-standard training program associated with an ACGME-accredited program, or hold an unrestricted license to practice medicine in a U.S. state, U.S. territory, or the District of Columbia. Once you fulfill this requirement, your ECFMG certificate will be valid indefinitely, meaning it will no longer be subject to expiration. If your ECFMG certificate is close to expiring (for example, 2024 Pathways certificates expire on December 31, 2026, meaning you may start residency certified but become uncertified mid-year), you should contact ECFMG to request an extension of validity until you complete the requirements for permanent certification.
What happens if my ECFMG certificate expires before I match into a residency program?
If your ECFMG certificate expires before you match into a residency program, you may need to revalidate your certificate by applying for a new pathway. Revalidation is only possible through Pathways 1 or 6. So, if you’re not eligible for Pathway 1, you must apply through Pathway 6, which is considerably more difficult. Read more about pathways expiration here.
What is the seven-year rule for completing all USMLE exams for ECFMG certification?
The seven-year rule requires IMGs to complete USMLE Step 1, USMLE Step 2 CK, and fulfill the clinical skills requirement through one of the ECFMG pathways within seven years of passing their first USMLE exam. USMLE Step 3 is not required for ECFMG certification.
For example, if you passed Step 1 on January 1, 2020, you must complete Step 2 CK and meet your pathway requirements no later than January 1, 2027 to prevent your Step 1 result from expiring for ECFMG certification purposes.
What happens if I do not complete all USMLE exams within the seven-year period?
If you do not complete all USMLE exams and meet the pathway requirements within the seven-year timeframe, your exam scores will no longer be valid for ECFMG certification. This means you may have to retake the expired exams and restart the certification process. In some cases, you may contact ECFMG to determine if your scores can be extended, but this is not guaranteed.
IMPORTANT 2027 reminder: ECFMG explicitly states that it will NOT notify you of upcoming seven-year deadlines or warn you when a passing performance becomes invalid. Tracking your timeline is 100% your responsibility. If you passed Step 1 in 2020 or 2021, this is the cycle to watch. Don’t assume ECFMG will email you a heads-up before your scores expire.
When is the recommended time to obtain my ECFMG certificate if I plan to apply for the upcoming residency match?
The deadline to submit your 2027 Pathways application is January 31, 2027 (Eastern Time, U.S.) for MATCH 2027. All required documentation, including your Certificate or Letter of Good Standing (Pathway 1) and OET Medicine scores released to ECFMG, must also be received by ECFMG by January 31, 2027.
However, it is highly recommended to apply much earlier so you obtain your ECFMG certificate before the ERAS application becomes visible to program directors, which is September 23, 2026, for MATCH 2027 applicants. Having your certificate in hand by this date ensures that program directors can see that you are fully certified and eligible for residency training. Keep in mind that some programs filter IMG applicants based on whether they already have an ECFMG certificate.
How does certificate expiration impact my ability to participate in the NRMP Match?
To participate in the NRMP Main Residency Match, you must meet all ECFMG certification requirements before the NRMP Rank Order List (ROL) certification deadline, which typically falls in late February. If your ECFMG certificate expires before this deadline and is not revalidated or extended, you will not be eligible to participate in the Match.
What is the cost of ECFMG certificate?
The application fee for a Pathway for ECFMG Certification is $945 USD (increased from $925 in the 2026 cycle). Full payment is due when you submit your application, and the fee is non-refundable, even if your application is rejected. Payment is accepted by Visa, MasterCard, Discover, or American Express only.
How long does it take to receive an ECFMG certificate?
It typically takes about two weeks for ECFMG to issue a certificate once all the necessary requirements have been successfully completed. Try to upload all the required documents at the time of submission since processing time takes about 5 business days at each stage (initial review, additional documents, final review), so you do not have to wait an extra 5 days for additional document processing.
